1) Create a new PSD document sized 250x150px. Draw a rectangule sized like you see below with Radius value 10px with Rounded Rectangule Tool. Drag Anchor Points (near T icon) with Direct Selection and when they’re on control line, right click and create 4 new point which pointed with arrows by Add Anchor Point. After general modelling is done, right click on the layer of rectangule and apply Rasterize Layer.  2) Open the Layer Style which is located under Layer menu, and apply the processes you see below.
Drop Shadow Settings  3) Bevel and Emboss Settings  4) Texture Settings, here you will prepare the Texture shaped like L your own. We told how to do it in "Square Boxes On Picture" sample.
